The Service will be delivered and based in London Borough of Tower Hamlets and managed locally. The new service is expected to be a seven-day service provided to prevent Delayed Transfer of Care for older people medically fit for discharge by offering practical support and assistance to leave hospital. The scheme operates Monday to Sunday and is available to people aged 65+, but will prioritise those who are 50 and over who are isolated or at risk of readmission.
