Description

North East London CCG are seeking to commission a community based service providing high quality specialised lymphoedema and complex wound management. There is also additional elements of dressing optimisation and lymphoedema hosiery management and supply to patients, primary and community health and social care providers. The service will be innovative, accessible, integrate and align with providers within the health and social care economy. The provider will be required to deliver a specialist clinician led service that provides lymphoedema and complex wound care to patents across North East London within the boroughs of Barking & Dagenham, City and Hackney, Havering, Newham, Redbridge, Tower Hamlets and Waltham Forest. The service will need to provide services within the boroughs at suitable locations. The aim will be to: • Deliver a lymphoedema service and additional complex wound care that provides innovative and effective care at the point of need, thereby reducing the chronicity associated with unmanaged lymphoedema or non-healing wounds. • Ensure patients are seen and treated in the right place at the right time and by the most appropriate person/service. • Ensure patients have equal access to consistent levels of service regardless of where they live and their gender, age, ethnicity, disability, sexual orientation, religion or beliefs. • Reduce the number of inappropriate attendances at emergency and urgent care and increase the management of patients locally. • Develop common pathways of care ensuring there is no unnecessary duplication and promote the coordination of services. • The service will promote equitable provision and access to excellent wound care and chronic oedema management for patients and contribute to a better patient experience and quality of life. • The service will deliver training to local services across NEL of healthcare professionals improving the standards of care provided for lymphoedema and wound care.
