Description
NHS Hull Clinical Commissioning Group is seeking to commission a proof of concept Homeless Discharge Service through an open procurement procedure. The service will deliver a clinically safe, high quality, dignified, and cost effective service that puts the individual at the centre of their own care and work to transform health outcomes for one of the most vulnerable and deprived groups in society. The service will be required to: • Provide a healthcare professional led model of integrated healthcare for homeless people who are admitted to the local acute hospital, Hull University Teaching Hospitals NHS Trust (HUTHT); • Support the delivery of sustained health improvement of individuals who are homeless through provision of support on discharge; • Provide pro-active and personalised care to people to homeless people; • Improve access to appropriate health and social care services for people experiencing homelessness; • Improve the experience of health and social care services for individuals who are homeless; • Liaise and work with key local partners, stakeholders and services to support the health and care interventions provided to individuals who are homeless.
