Description
The London Borough of Hounslow strategy 'Joint Prevention Strategy for Adult Services in Hounslow 2015-19' focuses on promoting independence for those at risk of or already using health and social care services. To support the strategy London Borough of Hounslow decided to commission Integrated Advocacy Service that will empower Hounslow residents to express their views, make informal decisions regarding their lives and assist them to achieve their rights and entitlements. Scope The Integrated Advocacy service will support vulnerable individuals to: • live independently for longer in the community and ensure they are able to maximise their independence, choice and control and empower them to actively participate in the community • have easy access to information, advice and signposting to enable them to make decisions about their own care and support needs. The Integrated Advocacy service the following statutory advocacy functions: Element A - Independent Mental Health Advocacy service (IMHA) Element B - Independent Mental Capacity Advocacy and Deprivation f Liberty Safeguards Relevant Person's Representatives (IMCA and DOLS RPR) Element C - Independent NHS Complaints Advocacy A single provider will be commissioned to deliver the Integrated Advocacy service.
