Description
Leeds is one of the four areas nationally being supported by NHS England in a development programme on Population Health Management. It has been agreed that £25k from NHS England will be available for each of the four LCPs to design and test interventions to support people living with frailty. The allocation of this funding has been agreed by the West Yorkshire ICS. The intervention is aimed at identifying and working proactively with patients living with moderate frailty within the Pudsey & Bramley areas. Our approach is strength based, focussing on working with patients to increase their resilience and address social factors which may impact upon their health. Once identified we will contact the patients and invite them to a "Living Well" group session, which will be run by the OT team, in conjunction with a PCN pharmacist, a social prescriber and 3rd sector agencies. This will be followed up by further interventions with the "Living Well" nurse and signposting as appropriate.
