Description
The London Borough of Bromley is seeking tenders from suitably qualified providers to provide a specialist community based support service for Service Users, their family and carers following a diagnosis of dementia. This includes people under 65 who are employed and people over 65 at the Early Stage (Moderate Decline) to Mid-Stage (Moderately Severe Decline) who may then move into statutory specialist provision. The Service must meet the individual needs of each Service User and be flexible and proactive in meeting those needs in partnership with the Service User and other key stakeholders working with them. The aim of the Service is to reduce and delay the need for more costly or intensive services and maintaining Service Users independence as they move along the dementia pathway.
