0-5 HEALTHY LIFESTYLE SERVICE

Public Health Hackney would like to commission a provider to provide 0-5 Healthy Lifestyle Service. The service shall deliver universal and targeted 'healthy lifestyle' services for children aged 0-5 and their families in City and Hackney with the overall aim of preventing and reducing levels of unhealthy weight in the 0-5 population. The service will deliver a range of multicomponent, evidence-based interventions for City and Hackney as part of the following four overarching key functions: 1. Promotion and distribution of Healthy Start Vitamins and targeted promotion of the Healthy Start Scheme 2.Healthy lifestyle education workshops for families 3.Health promotion of healthy weight 4.Training and development The provider shall work in close partnership with City and Hackney partners including Hackney Education and early years services, Health Visiting, Family Nurse Partnership, Homerton Hospital Dietetics service and primary care services in City and Hackney to support 0-5s and their families.
