Description
East Riding of Yorkshire Clinical Commissioning Group (ERY CCG) sought to commission an ambitious programme of work to redesign the way in which Community Services are delivered across its geography. In doing so, the CCG has developed a clear vision of the outcomes it wishes to achieve for its population, ensuring that services are delivered in a more seamless and integrated way that is closer to people's homes. The service will adopt an outcomes based approach to innovation and transformational ways of working, deliver greater partnership working from a range of providers across the care economy and Commission from a single Prime Provider that will be accountable for the delivery of all in scope services and related patient outcomes. The Contract will be awarded for an initial period of five years, with an option to extend for a further two years at the CCG's discretion.
