Description
The Support to Return Home from Hospital service provides support to Birmingham citizens being discharged from hospital or enablement bedded setting, as well as contributing to their timely discharge. Adults who live alone without any other means of support could receive a range of support for up to 6 weeks to enable them to return home and remain there thus also preventing readmissions and strain on other services in the vital period of time post discharge. This will include delivering the following: • Support on the day of discharge for individuals with follow-up support within 72 hours • Deliver a basic food parcel • Befriending • Undertake simple shopping (non-financial) • Making sure the home environment is warm, comfortable and safe • Supporting to pay bills and make appointments • Collecting medication prescriptions or setting up a prescription delivery • Practical assistance with accumulated posts, completion of forms, letter writing and posting • Helping to find out about and access community activities to keep people connected to their local communities
