Description

Service providers will implement a model of service delivery based on 12 units of Stage 2 accommodation of the staged model. Accommodation must be within the Winchester City Council boundary and the provider must work in partnership with other providers of local services including but not limited to other social inclusion services, drugs and alcohol inclusion services and community mental health services. Stage 2 accommodation-based support will provide less intensive support, helping individuals to realise their independence. The model implemented will meet the diverse needs of people in need of housing related support and service providers will develop innovative solutions that maximise the use of resources, avoid duplication, reduce bureaucracy and unnecessary process, and improve service access. Services will deliver holistic support that enables service users to manage crisis, avoid eviction and repeat homelessness, facilitates engagement with other services as required and reduces the likelihood of crises happening in the future. Service providers will be required to target services and provide personalised support to meet a wide range of needs. Providers will be expected to proactively manage expectations of service users whilst supporting them to achieve health, wellbeing and wider social inclusion outcomes. Services will act as a catalyst for improving inter-agency approaches and partnership working to meet the needs of people who are or who are at risk of becoming homeless and will deliver against shared outcomes across statutory and voluntary services. All services will provide flexible and responsive support to vulnerable people who are in need of support because they are homeless or at risk of becoming homeless. This may include but not be limited to, people with substance misuse issues (including dependency and co-occurring conditions), mental health problems, physical disabilities or long-term physical health conditions, learning disabilities (where service users do not reach the threshold for adult social care) and those with a history of offending behaviours.
