Material Supply & Associated Services Aids & Adaptations

The Contract Title: Lot 4 Aids & Adaptations Items being Supplied: Adapted plumbing supplies and associated products e.g. wet rooms, shower trays, adapted showers, brassware, mobility support, Doc M packs, general disabled adaptations. The form of Contract is PFH Call off Contract The Contract Period is : 1 year with the option to extend by a further two years in 12 months increments total if all extension taken 3 years subject to satisfactory performance reviews. Providers & Contract value ProCare, £400K Per annum The contract will be backed up by Service Level Agreements (SLAs) and Key Performance Indicators (KPIs).
