Bespoke Care Package for a Young Person in Peterborough

Provision of a long-term, bespoke care package within a domestic home setting, for a young male aged 13 with moderate learning disability, autism-spectrum disorder (ASD), attention-deficit hyperactivity disorder (ADHD), and behaviour that challenges (the client). The client is currently residing and being cared for within a hospital which is outside of the immediate area of his family. The CCG wishes to commission a new care package solution involving a discharge close to family ties within Peterborough. The solution is looking likely to involve the adaptation of a private residential dwelling to accommodate the client and his team of carers. The new home is being designed with the specific client's needs in mind and will include secure outdoor space. The new domestic setting will be managed and maintained by a housing association under contract with the CCG, with the additional support from the Local Authority and NHS England and Improvement.
