Description
North of England Commissioning Support (NECS) on behalf of NHS Cumbria Clinical Commissioning Group undertook a procurement for the provision of an Elderly Care Coordinator Service for the Copeland Locality. The Copeland Elderly Care Coordinator Service is a patient-centred service that empowers an individual to take control of their own care and support plan. Families and carers are also involved in the care planning process. The key objectives were: Improved patient experience; Reduced impact on Primary Care; Reduced impact on Secondary Care; More effective use of Community Services; and Better Medicines Management The service was to commence from 1st August 2015 with contracts to be awarded for a period of 8 months. The service was procured using a Lots approach, with each GP Practice (9 in total) being identified as a Lot. Bidders were only able to apply for one Lot.
