Early Intervention Link Workers Support Service

The London Borough of Hammersmith and Fulham (LBHF) intends to procure a 3-year Contract for the provision of early intervention for children and young people who are at risk of poor life outcomes. It is expected that the service will be delivered through Link Workers who will support, advise and signpost the children and young people to appropriate services within LBHF. The Borough's preferred option is to award the contract, to run from September 2016 to August 2019, to a single service Provider who would cover the range of support functions. It is expected that the successful Contractor will source additional funds through a Social Investment model.
