Description
The new Independent Living Service (Mental Health) service will continue to provide outcomes focused lowlevel housing related support to enable vulnerable people to develop the skills needed to gain and maintain atenancy, and live independently in the community. Housing related support consists of non-personal care based support, to assist people to: • support people to remain independent in the community • support people to obtain, manage and maintain their accommodation to ensure it meets their care and supportneeds • provide appropriate information and advice to support people to make informed decisions about current andfuture needs • prevent or delay people's health or social care needs from becoming more serious • reduce the likelihood of people being admitted to hospital or residential care and support hospital discharges • create social capital and enhance personal resilience • support people to ensure their income is maximised
