Description
The service will provide on-demand video based clinical consultations which will be available 24/7 The TMS Includes: - Initial clinical assessment by an appropriately trained registered practitioner - Escalation to other specialist clinical resources (e.g., an appropriately qualified consultant, medical registrar, or specialist nurse) where required - Coordination with local clinical resources (district nurses, community teams, ambulance service and the resident's GP) where required - 24/7 clinical assessment and support for all residents who fall to reduce attendance at ED and acute admission - Provision of virtual meeting room to facilitate community service providers and other relevant partners to conduct their MDT meetings - Consultant Pharmacist support on prescribing, deprescribing, mentoring/training, medicines safety and research. Input to developing services responsive to local needs and available as an expert resource. - Clinical support for safer implementation of Medicines Optimisation - Support for the DES Requirements of the Enhanced Health in Care Homes Initiative The principal mechanism for consultations will be video but voice calls and other forms of communication will be used where necessary. All consultations will be recorded via SystmOne into the patient record and a copy of this will be supplied to the GP. Each clinical consultation will entail: - Initial assessment of the person's condition by a registered practitioner - Escalation to a suitably qualified consultant, medical registrar or specialist nurse if required - Administration of a prescription if required - Referral to and/or coordination with the persons GP, district or community nursing team, ambulance service to include any phone calls or video calls required - Provision of advice and guidance to the persons local carers - Clinical supervision of any procedures or other activities undertaken by the local carers at the request of the clinical assessors - Follow-up calls by the TMS team to check on the person's progress - Creation and updating of the electronic patient record to include the TMS clinical consultation and the supply of a copy of that record to the GP. If required CH staff will be offered clinical supervision by the TMS to perform basic procedures.
