Description
Wigan Council commissions a number of health improvement services that work with the individual to identify their own aspirations and goals through a holistic person centred approach. The providers of health improvement services will ensure that the individual accesses the most appropriate expertise as required by utilising a flexible multidisciplinary approach across services. There is an expectation that health improvement services work collaboratively together to improve health outcomes. Lot 1: Health Improvement Service To provide an integrated health improvement and wellbeing service for the population of Wigan that can support residents to improve and manage their health and wellbeing this includes providing smoking cessation interventions. Lot 2: Specialist Weight Management Service To provide a Specialist Weight Management Service with the aim of achieving the best health and well-being outcomes for individual. Lot 3: Community Link Worker Service To address non-clinical demands on primary, community and acute health care services by delivering a tested intervention that uses 'health coaching' and community knowledge to connect people to sources of support within their community. Community Link Workers interventions improve the health and well-being of local people by connecting them to community based activities which support their independence and reduce reliance on acute or specialist services, with a focus on early intervention and prevention.
