Description
This contract is a call-off contract from the Help To Live At Home Open Framework (AS08860). The call-off contract relates to the provision of social care services within a newly purpose-built supported living scheme (known as a "Romsey Extra Care - Nightingale Lodge" service), located within the town of Romsey in central Hampshire. The commissioning and procurement of this service contributes to several of the County Council's Adults Health and Care 5 Year Strategy Business Plan (BP) areas, e.g. BP13 (Older Person's Commissioning) and BP 11 (Older Persons Operations including Hospital Discharge). The call-off contract being tendered is intended to reduce the department's reliance on the use of residential care by older people and to maximise the opportunities for older people to remain living independently within their own homes. Care and support will be provided within a purpose built, accessible by design, "extra-care" environment to support residents to remain as independent as possible for as long as possible. The service will: • Respond quickly to residents changing needs • Provide unplanned care as and when required in addition to planned care • Provide an emergency response The service will support individuals to have as much choice and control of their lives as possible, and to maximise their skills within their capabilities. The accommodation for the service is newly built and is owned by People For Places Living Plus. The accommodation comprises 54 self-contained flats of mixed tenure, 17 being for shared ownership and 37 being for rent. The rental accommodation will be leased to residents by Places for People Living Plus, a Registered Provider of Social Housing.
