Description
The Council is looking for providers to deliver the Making Connections Walsall Programme. The MCW programme is about tackling loneliness and social isolation, improve wellbeing and reduce preventable health and social care service usage. It will take a Social Prescribing approach to improve the social connectedness of older people across Walsall. The Making Connections Walsall Programme is targeted at meeting the social needs of the older adults who are lonely and socially isolated NOT SOCIAL CARE or MEDICAL needs. Providers with a local understanding of Walsall's Older population and experience of successfully supporting vulnerable older people to address their social needs and access community initiatives. Making Connections Walsall will be delivered through a hub and spoke locality model connected through a consortium of hubs. There will be four consortium hubs each of which will be responsible for connecting lonely older people to other appropriate community based provision. We are seeking for a rich complement of individual provider organisations across the Borough and therefore will not be considering consortia bids. The four Hubs will be delivered by different organisations and will cover the following areas: Hub 1: North Walsall - AP 3 - Bloxwich East, Bloxwich West, Birchills Leamore, Blakenhall Hub 2: West Walsall - AP5 - Bentley and Darlaston North, Darlaston South, AP6 - Willenhall North, Shorth Heath, Willenhall South Hub 3: East Walsall - AP1 - Pelsall, Brownhills, Aldridge North, Rushall-Shelfield and Walsall Wood, AP2 - Aldridge and Beacon Hub 4: South Walsall - AP4 - Walsall South, St Matthews, Paddock, Palfrey, Pleck
