This is a market engagement exercise. **** Previously published under SOL - 14767. Some providers had issues reviewing documentation, therefore we have to re-publish. The previous project will close tomorrow morning, so please can you re-submit your response under this project!*** Care at Home services, or domiciliary care, supports residents with care and support needs to remain in their own homes, whilst receiving support with their activities of daily living. These include getting up / going to bed, washing dressing, eating and drinking (and food preparation) and toileting. Hospital Discharge Services support people to be discharged from hospital on the day they are medically fit to leave and provide support term support for up to 6 weeks to regain independence skills and support with their activities of daily living. The main of the service is to support as many people as possible to go home from hospital, rather than into bed-based care, and to maximise people's longer-term independence. Admission avoidance services (known in Solihull as Early Response Service) supports people when they first present to hospital or to the social care front door, providing short term reabling support (up to 6 weeks), preventing the need for admission to hospital or long-term care services. The service provides support to help people live as independently as possible.
