Description
Kirklees Council wish to commission a Dementia Information and Advice Service (DIAS) which delivers and develops: • Dementia Friendly Communities - pre-diagnosis • Information and Advice - post-diagnosis The DIAS will work to raise awareness and understanding within communities in Kirklees pre diagnosis, signposting onto the diagnosis pathway where appropriate; and will offer all individuals being diagnosed with dementia access to a set of information and advice sessions. This Service will operate closely with South West Yorkshire Partnership NHS Foundation Trust who deliver the dementia diagnosis pathway across Kirklees. It is anticipated that the Contract is to commence on the 1st October 2017 and will expire on 30th September 2019, with an option to extend for a further period, or for successive periods, of up to three years. Tenderers should note that the contract sum, for delivering the requirements of this service, is fixed at £155,000.00 per annum.
