Bromley CCG Integratyed Specialist Diabetes Service

The overall aim is to provide more relevant and active management of patients with Diabetes, ensuring that the Diabetes care that can be appropriately managed in primary care is done so in this setting to ensure that patients are not inappropriately admitted to hospital, via A&E, and that patient experience and clinical outcomes are improved.
