Description
The service will enable and support people to live as independently as possible in their own homes. The care and support will have a person-centred approach that enables individuals to 'live a good life' by using assistive technology wherever possible to promote independence. This contract is for 2 element of the Help to Live at Home: 1. Home Independence and Crisis Support, which includes hospital to home, intermediate care, restorative support, hospital prevent, emergency response 24/7 and crisis support. 2.Home Independence Service, which includes long term support, palliative/end of life support and night support. Due to the urgency to replace this contract and ensure service continuity, the council undertook a Open Procedure restricted to 2 suppliers with the capability and capacity to deliver the contract. The award of this short term 15 month contract will allow the Council to carry out a PCR15 compliant procurement for a longer term contract.
