Description

Torbay Council wishes to redesign and commission some of its existing services to create a new Integrated 0-19 years Service for Children, Young People & Families in Torbay. The Authority is looking to commission a new service for April 2019. The current services included in this plan are: • Health Visiting; • School Nursing; • National Child Measurement Programme; • Children's Centres; • Early Help Co-ordination; • Team Around the Family Co-ordination; • Family Intervention Team; • Young People's Substance Misuse Service; • Advocacy and Independent Visitors Service; • Missing and Return Home Interview Service. The Authority wants to explore a model that delivers these services altogether through 'Family Hubs'. The Authority requires Family Hubs to deliver holistic, early intervention services to Children, Young People and Families. Further details on the Integrated 0-19 years Service for Children, Young People & Families in Torbay and the dates and details for the proposed provider engagement/workshop events, is included within Appendices 1, 2 and 3.
