Leeds HIV Social Care Support Service

The service is for the provision of a citywide Leeds HIV Social Care Support Service that will work within the principles co-production, person centred and outcome focused support for individuals aged 18+ (17+ if they are transitioning from Children's/Families Services), living with HIV and residing in the Leeds Metropolitan District. The service will need to have an accessible Leeds city centre base and will operate 52 weeks of the year, excluding bank holidays. The service aims to focus on the principles of sustaining choice, involvement and inclusion that facilitate opportunities for individuals with HIV to maximise their lives.
