Description

A community based ophthalmology service that will provide an integrated and cohesive approach to delivery of the triage, community assessment and treatment and low vision aids. The Service will offer responsive, accessible and high quality ophthalmology assessment and monitoring outside of the secondary care services, with robust clinical oversight and formalised arrangements for continuous quality improvement. The Community Ophthalmology Service will provide the following community services: • Community Triage (GOS18 and GP referral • Community Assessment and Treatment • Community Low Vision Aid Service The aim of the Community Ophthalmology Service is to manage patients with ophthalmic conditions in a primary care setting where it is appropriate to do so. The Service will ensure that patients are transferred to secondary or specialist care only when specialist treatment is required.
