Description

The main objectives of this pilot are to introduce and build the foundations for work strand 4 of the South Tees Better Care Fund: Support to Care Home Project. Through the implementation of a revised and improved falls protocol for residential care homes; coupled with the implementation of the use of Telecare. The expected outcomes include: • Reduction in non-elective admissions caused by falls as a result of improved education and management of falls • Reduction in falls related safeguarding alerts • Improved patient experience • Reduction in the number of North East Ambulance Service call outs to respond to falls within residential homes
