Description

The ADI MyPathway solution is currently actively being used across the Muscoloskeletal (MSK) service by 40,827 users, as well as 124 in the Motor Neurone Disease (MND) service. This is an expansion of the MyPathway app across the Trust. The patient app provides an electronic contact point between the patient and our services. It is personal to the user and allows them to interact with their care teams around details of their condition, care and appointments in a digitally secure environment. The app provides access to personal electronic communications, instructional videos and resources, and links to pre- and post-appointment questionnaires. The project aim is to deploy ADI MyPathway across multiple departments, settings, and scenarios within Sheffield Teaching Hospitals NHS Foundation Trust. This will be achieved by extending the product with tools to allow user-driven customization and flexibility according to the needs of the different settings. The following 'pillars' of key functionality are identified, and the project will be delivered in phases approximating to these. #1 Ability to communicate with all patients #2 Case-load management #3 Portal for PIFU and FU PB #4 Patients have Visibility of location of all in-hospital appointments #5 Self-configure/alter 5 automated pathways
