Description
PRIOR INFORMATION NOTICE Bracknell Forest Council is looking for a suitable provider to deliver a health visiting service to improve the health outcomes for the population of 0-5 year-old children and their families and to reduce health inequalities. The Health Visiting Service will consist of specialist community public health nurses (SCPHN) and skill mix teams who provide expert information, assessments and interventions to families and work in partnership with a range of different agencies. Health Visitors are responsible for leading and delivering the mandated and additional elements of the Department of Health's Healthy Child Programme for 0-5 year old children and their families.
