Domiciliary Care service for Adults eligible for NHS Continuing Healthcare incl. Fast Track

NHS Nottingham and Nottinghamshire Clinical Commissioning Group is inviting Expressions of Interest from providers of Domiciliary Care Services for Adults eligible for NHS Continuing Healthcare including Fast Track. The service will be provided for people (Service Users) who have been determined by the Commissioner as eligible for NHS Continuing Healthcare. The objective of the service is personalised care that is safe and promotes a good quality of life, meets assessed health and social care needs and contributes to the outcomes identified for each individual Service User. Also, to contribute to the reduction of inappropriate hospital admissions where patients have expressed a wish to be cared for within their own home.
