Description
The aim of the Community and Lifestyle Weight Management Service is to improve the health and wellbeing of overweight and obese adults and young people aged over 14 (with parental consent) by supporting them to achieve and maintain a healthier weight and improve physical fitness. The aim is do this through helping service users make appropriate and sustainable lifestyle changes to their eating and physical activity habits. This forms part of the Integrated Wakefield Weight Management Service pathway. There is a requirement to commission services to provide a range of interventions to support people across the Wakefield District. Commissioned services within the Community and Lifestyle Weight Management Service specification are based on lifestyle and community weight management programmes. These are what are usually referred to as Tier 2 interventions. The provision of interventions as part of this service must be focused on a lifestyle and behaviour change to support sustainable change, self-efficacy, weight loss and prevention of weight gain.
