Description

We are procuring a care provider to operate bed-based intermediate care services at St Thomas', delivering short-term recovery, reablement and rehabilitation for adults with complex needs. The provider will deliver regulated residential and nursing care within a purpose-built facility and work as part of an integrated model alongside health and social care partners. This is required to improve hospital flow, reduce avoidable admissions, and support people to regain independence and return home. It responds to growing demand linked to frailty and ageing and replaces fragmented provision with a modern, integrated and financially sustainable model.
