The Local Authority is committed to the provision and offer of high quality day time support Services to adults and older people (aged 50 years or over), who are eligible for adult social care support. Short-Term Support including Outreach: These Services are intended to provide appropriate support and advice for a limited period of time to allow for an individual's specific needs to be assessed, a support plan to be developed and for short term interventions/activities to be put in place. This type of day time support may follow reablement or link with specialist social rehabilitation support. This short term support may be provided within a building base or via outreach in the community. The individual taking part may therefore access this support directly from their home without physically attending a building base. Outreach Support: This is support to enable customers to access community based opportunities either on a one to one or group basis. Support provided at the usual building base or the customers own home cannot be classed as outreach. Outreach can help individuals' to identify their needs, aspirations and wishes and provides support to maintain and develop links with their local community through the use of local activities or resources to support people's connections and relationships. It is expected that at least 40 hours per week of these day time support Services will be delivered as community based support or outreach in the community. Medium and Longer Term Support: This service response aims to meet the care and support needs on a medium and longer term basis for people who are particularly dependent or have complex and sometimes challenging needs and behaviour/s. For some people with physical needs associated with disability and/or frailty opportunities to maximise regaining or maintaining maximum levels of independence will be provided. Carer Breaks: Day time support Services are also to provide support to informal or family carers by providing good quality, flexible breaks for carers.
