Description

Durham County Council holds the budget for the regional Integrated Care System (ICS) Prevention Board, which is to be used to fund the employment of a full time NHS Band 4 Recovery Navigator for 12-month period. This post will be employed by North Tees and Hartlepool Foundation Trust, Hardwick Rd, Hardwick, Stockton on-Tees TS19. The service objectives are: • To reduce the impact of alcohol related frequent attenders including include those affected by alcohol-related violence. • Enhance the Alcohol Care Team by providing additional patient centred support between acute and community services • Ensure that service transitions are the norm and are increasingly effective The post holder will deliver and contribute to the development of an innovative recovery programme which will offer a range of support to address what matters to the client, e.g. housing, benefits, welfare, etc. The post would support secondary care clinicians; including those in ED to support service users into support within the hospital and, crucially, post discharge. This challenging role will involve delivering practical, bespoke interventions both in person and through a wide range of network partner agencies that support people away from substance misuse and violence, towards a safer, healthier and more productive lifestyle. It is envisaged that the post holder will work as an extended member of commissioned drug and alcohol services in the community and in addition to improve the patient journey will also report back to the partnership group where potential challenges and bottlenecks are in the current system. The Navigator will also explore the volunteer role working closely with the recovery community to recruit previous service users who are able to relate and show compassion to patients showing instant trust and sustained engagement with treatment programmes and support outreach visits
