Description
Bracknell Forest Public Health is looking to commission one provider to provide a Falls Prevention Advisory Service. The service will be available to all Bracknell Forest residents aged 65 and over. The adviser(s) will be trained professionals able to carry out a detailed assessment of falls risk within the home with a view to designing (in collaboration with individuals and families) a package of community based support and advice. The service will be beneficial for any individual who is assessed as being at a 'medium risk' of falling as defined by the outcome of a standardised screening tool (based on existing accredited risk assessment tools). The service aims to prevent falls before they occur. Individuals may self-refer or be referred by a professional or agency. A Strength & Balance Service This service is a rolling 12 week programme of strength and balance classes that are available to Bracknell Forest residents aged 65 and over.
