Description
The London Borough of Hammersmith & Fulham (the Council) is inviting suitably qualified and experienced providers to tender for the delivery of Community Reablement & Home Care Services. This procurement supports a continuum of support available to residents to regain or retain their independence living in the community. The services will provide support for adults in Hammersmith & Fulham by proactively taking all opportunities to build on people's strengths, maximise their independence and connect them with their community. This procurement represents a strategic redesign of the previous service model. It reflects learning from previous delivery, stakeholder engagement, and evolving adults social care priorities. The new model will have a focus on reablement enabling support to residents to regain the skills, confidence and social networks to return to their previous levels of independence as far as possible and reduce the likelihood of admission/ re-admission to hospital, or the need for long term care and support. Scope of Services: The commissioned services will provide intensive, enabling support to residents to regain the skills, confidence and social networks to return to their previous levels of independence as far as possible and reduce the likelihood of admission/ re-admission to hospital, or the need for long term care and support. While many people will not need support after reablement, some people will require further support to enable them to continue to live safely at home. The new model will take a more enabling approach to homecare, thereby increasing opportunities for further independence through closer relationships with our community health and primary care services to ensure early intervention when there is any deterioration or concerns with health conditions.

Participation
Conditions suppliers must meet to bid.
CQC Rating Potential Providers must be registered with the Care Quality Commission (CQC) for the provision of personal care and must hold a minimum overall CQC rating of “Good”. Potential Providers must also ensure that any sub-contractors who they engage to provide regulated activities (as described in the Specification) are required at all times to hold and maintain an “Outstanding” or “Good” CQC rating. Financial Capacity Potential Providers and all Subcontractors will be assessed for financial capacity. If the Potential Provider has a poor credit rating (which is a score of 50 or less in a CreditSafe check) and further assessment of the supporting documentation highlights concerns over financial viability that cannot be reasonably satisfied, then it is likely that the bid will be excluded.
Technical Ability Potential Providers must demonstrate relevant technical experience in delivering regulated Reablement and Homecare services that reflect the scope, scale and complexity described in the Specification. For Lot 1 – Community Reablement, to meet the minimum standard for the conditions of participation, Potential Providers must provide evidence of either: Option A (Single Contract): One (1) current Community Reablement contract with a Local Authority or NHS body in which the Potential Provider received a minimum value of £250,000 in the 2025/26 financial year; OR Option B (Multiple Contracts): Up to a maximum of three (3) current, Community Reablement contracts with Local Authority and/or NHS bodies, which, when combined, means that the Potential Provider received a minimum value of £390,000 in the 2025/26 financial year. For Lot 2 – Homecare, to meet the minimum standard for the conditions of participation, Potential Providers must provide evidence of either: Option A (Single Contract): One (1) current Homecare contract with a Local Authority or NHS body, in which the Potential Provider received a minimum value of £1.5 million in the 2025/26 financial year; OR Option B (Multiple Contracts): Up to a maximum of three (3) current Homecare contracts with a Local Authority and/or NHS bodies which, when combined, means that the Potential Provider received a minimum value of £2 million in the 2025/26 financial year. Providers are required to demonstrate the actual monetary value received (i.e. income invoiced and/or paid) from Homecare or Community Reablement contracts delivered during the 2025/26 financial year. Potential Providers and any sub-contractors must demonstrate that they have a CQC registered operational office or delivery base that has a minimum CQC rating of “Outstanding” or “Good” located within the London Borough of Hammersmith & Fulham or in one of the Boroughs that share a boundary with Hammersmith & Fulham. This includes: · Royal Borough of Kensington and Chelsea · London Borough of Brent · London Borough of Ealing · London Borough of Hounslow · London Borough of Richmond upon Thames · London Borough of Wandsworth All Conditions of Participation relating to technical ability, regulatory compliance and local operational presence have been set in direct alignment with the requirements of the Specification to ensure that appointed Providers are capable of delivering the Services to the required standard. Where minimum standards are specified in relation to technical ability, regulatory compliance, or local operational presence, these will be assessed on a pass/fail basis as Conditions of Participation. Potential Providers failing to meet these minimum standards will be excluded from the procurement process.
