Description
The Dementia Navigation, Information and Support Service supports City & Hackney residents aged 18 years and over with dementia, their carers', families and friends. The purpose of the Dementia Navigator service is to enable people diagnosed with dementia, and their carers, to come to terms with their diagnosis, reduce social isolation and live well with dementia. The high level outcomes expected for people with dementia and their carers are aligned to the expectations contained within the NHSE CCG assessment and improvement framework on dementia care planning and post diagnostic support. The service will be required to comply with any relevant updated NHS guidance on dementia care which is current. The service ensures that people with dementia and their carers are supported in primary care, without replacing the primary care service, and supported about learning about dementia, its progressions and ideas for self-management and peers support. The service ensures that people with dementia and their carers will remain independent, improve the wellbeing and have opportunities for social engagement to increase self-confidence a self-sufficiency through the progression of dementia. The Dementia Navigators are responsible for supporting patients through the dementia pathway from pre diagnosis, diagnosis, discharge and planning support. The Dementia Navigators provide support to Care Homes where patients are reviewed every 6 months. Additionally, the Dementia Navigators will assess and provide support to create a community support plan to address unmet needs of the patients. The Dementia Navigators will provide ongoing support to those patients and ensure patients are engaged, prevent social isolation and prevent admission to hospitals and care homes. The provider of the Dementia Navigator service does not need to be a statutory health care or social care provider however the provider will need to have good links with General Practitioners, Acute Hospitals, Mental Health Trusts and Care Homes in the City & Hackney area. The provider will also need to seek the appropriate accreditation (e.g. CQC registration) where this is requirement of practice. The CCG's current contract for the Dementia Navigator service is with the Alzheimer Society which will end on 31st of March 2023. The CCG is publishing this notice to investigate if there are any other organisation that may be interested in providing the service before the CCG decides on options for re-contracting for the service.
