Description
Warwickshire Institute for Diabetes, Endocrinology and Metabolism (WISDEM) Centre based at UHCW acts as a tertiary centre for Coventry and Warwickshire and for the regional clinical network as well as a secondary centre for Coventry and Rugby CCG. The vascular hub for Coventry and Warwickshire is also situated at UHCW. As a consequence the MDFT based at WISDEM Centre, UHCW frequently provides local and regional care to the most complex patients (at risk of life, limb or tissue loss due to their vascular or diabetes complications such as sepsis, gangrene, critical limb ischaemia, end stage renal failure and neuropathy). Patients who cannot be successfully managed within the standard orthotic pathway are to be referred to the Specialist Vascular/Active and High Foot Risk Orthotic Patient Service. This service is to provide a bespoke service to patients who have had or are at very high risk of lower limb/foot amputation or chronic ulceration and/or critical limb ischaemia they frequently present with multiple co-morbidities, osteomyelitis and/or complex bio-mechanical deformities including charcot neuroarthropathy. This service will form an essential part of the Multi-Disciplinary Vascular and Diabetic Foot Teams. These teams work closely together to optimise care and improve the outcomes of acute and chronic foot ulcerations and amputations.
