Description
The Integrated Commissioning Department of the Royal Borough of Kensington and Chelsea and the City of Westminster have awarded a contract to Age UK Kensington & Chelsea for the provision of an RBKC Older People's Preventative Service. The overall aim of the Older People's Preventative Service is to enable older residents of the Royal Borough of Kensington and Chelsea (RBKC) particularly those who are isolated, excluded and vulnerable to access services that enable them to live independently with improved health and wellbeing. The service will aim to ensure service users enjoy a good quality of life in their local community by having access to a range of services which aim to reduce the risk of social isolation and support the delivery of services which target the modifiable risk factors for dementia (depression, social isolation, lack of physical activity). The service shall help meet key public health outcomes including health related quality of life for older people (e.g. combating anxiety and depression), reducing excess deaths in winter through supporting food and fuel poverty initiatives and reducing hip fractures through exercise and mobility fracture prevention. The programme shall be delivered in four ways: 1) Older People's Main Preventative service ('Main service') will incorporate the following elements: - Exercise Provision - Community exercise sessions & exercise for the mind sessions - Health Promotion Activity - Travel Mentoring/Escorting - Intergenerational event & activities - Shopping Support - Small DIY Support - Befriending 2) Information and Advice ('I&A') service 3) Dementia 1:1 support service 4) Carers Alliance Service (CAS) The contract will be for an initial period of 18 months at a cost of £731,452, with the option to extend for a further 6 month period at a cost of £243,817. During this term, services will be redesigned following significant changes caused by the pandemic, and a competitive procurement exercise will be undertaken for future service provision which continues to meet the needs of residents.
