Description
The Council sought providers interested in delivering the Community Reablement at Home Pilot service in Hackney. The service provides Reablement support for people in their homes will be delivered by commissioned provider(s), with a focus on enabling individuals to achieve therapist-set goals over a period of up to 6 weeks.

Participation
Conditions suppliers must meet to bid.
Suppliers must have x1 turnover of £220,000.00 per annum. Suppliers must submit accounts for the past x2 financial years. Insurance thresholds are listed within the PSQ document.
Suppliers must have an in date and relevant Modern Slavery statement. Suppliers must have an in date and relevant travel policy. Suppliers must be able to demonstrate x2 relevant contracts delivering reablement services.
Supplier to have minimum turnover threshold of x1 £220,000.00 per annum. Supplier to upload x2 previous financial years accounts to PSQ. Insurance threshold will be specified on PSQ.
Supplier has to have an in date and relevant Modern Slavery statement. Supplier has to have an in date and relevant travel policy. Supplier must be able to demonstrate x2 relevant contracts of similar service delivery of reablement.
