Provision of MyWay Diabetes Platform for People Living with Diabetes

MyWay Diabetes is an evidence based self-management platform for people living with Diabetes. It provides patients with secure access to their medical records together with tailor advice, eLearning, data uploads, goal setting, The aim of this procurement is to ensure the seamless continuation of services from 1 April 2024, utilising the established integration and interoperability across North West London. The contract duration is 1st April 2024 to 30th September 2024. This has been awarded from Crown Commercial Services G-Cloud 13 Framework Agreement (RM15517).
