PHM development support (Central London Demonstrator Programme Provision)

The NHS NW London ICB required the provision of PHM development support (Central London Demonstrator Programme Provision). This contract aims to provide support and development to practically embed Population Health Management as an approach with Westminster's existing infrastructure and resources to underpin the integration of health and care services across the Borough. The programme of work will help the Borough team to improve outcomes for people with Long Term Conditions in Westminster. The Population Health Analytics reporting platform includes a linked data process and supporting data science engines, bringing together General Practice, acute, community, mental health, social care, and other data to support targeted interventions. The contract was awarded to Optum Health Solutions (UK) Ltd via a direct award on the G-cloud 13, Lot 3: Cloud Support Framework.
