Description
On 11 July 2018 the CCG awarded a contract with Age UK to provide a falls and post diagnosis dementia support service. The contract was awarded for 2 years with the option to extend for an additional year. This contract was awarded following an open procurement. In order to develop the falls and dementia service further the CCG wishes to pilot a social prescribing service and care coordination service in relation to this cohort of patients. This service can only be delivered by the existing provider as it needs to be embedded with the existing service. The pilot will be embedded and evaluated across Enfield before a decision is taken on future commissioning arrangements. This is part of developing a system of frailty care within Enfield. The contract start date will be 05/05/2019 and not the 06/05/2019.
