Description

Care closer to home programme aims to deliver integrated, community-based healthcare services which support people to stay healthy and live independently for longer. Enfield CCG, with our partners in health, social coal care and voluntary sector, will be aiming to deliver much more care in the community, either in patients' homes or closer to where they live and work. As part of the Care Closer to Home Enfield and its partners aim to provide better coordinated, holistic health and social care services across the health system for older people with frailty, emphasising the need for a greater focus on prevention, early identification and coordination of assessment, care planning and case management. Enfield CCG through the Better Care Fund Plan has proposed to ensure support and an ongoing point of contact for people with dementia and falls prevention is incorporated into the proposed development of the voluntary sector. The services provided by the voluntary sector will be crucial in supporting patients and their carers. Enfield CCG is seeking a lead organisation, to work in partnership with other voluntary sector organisations, to deliver a post diagnostic support for people with dementia and falls prevention service across Enfield. The provider will support the goal to promote better patient outcomes.
