Warwickshire CC wishes to commission a framework of providers to deliver a countywide adult weight management services and a single provider to manage, co-ordinate and deliver a framework of countywide providers for the delivery of universal and targeted physical activity/healthy lifestyles on referral for young people aged 12-16 and adults aged 16+. Lot 1 - The Delivery of the Universal and Targeted Warwickshire Physical Activity/Healthy Lifestyles on Referral Service for young People aged 12-16 and adults aged 16+ The provider will recruit, co-ordinate and manage providers of physical activities to meet differing service user needs and requirements to deliver a choice of activities, such as walking groups as well as leisure centre activities, across Warwickshire. The provider will manage and deliver a universal and targeted countywide physical activity/healthy lifestyles on referral service to young people (aged 12 to 16 years) and adults (16 years and above) who have been diagnosed with specific health conditions. Lot 2 - The Delivery of a Young Person and Adult Weight Management on Referral Service The framework of providers will deliver a countywide weight management on referral service to overweight and obese young people (12 to 16 years) and adults (16 years and above).
