Community Steady and Stable Falls Prevention Service - Hammersmith and Fulham, Central and West London

The Tri-borough Steady & Stable Falls Prevention service is an evidence based falls awareness and prevention service to; improve balance, reduce falls and keep people living independently. The service outcomes are ; • Improved functioning e.g. posture, balance, mobility, orientation and safety awareness • Prevention/reduction of unnecessary hospital admissions and other acute care as a result of a fall • Increased proportion of older people supported in their own homes • Enhanced choice and control over their lives
