Description
Contract for the provision of State of Care Services as follows; For the UEC emergency care pathway, we require • Provision of Contract Data relating to o access to primary care including 111, GP and other services, eg walk in centres o A&E attendance including transport to hospital, treatment and admission o Discharge following admission including any transfer of care and access to services that support a successful discharge o All comments must relate to a care provider • Minimum volumes to be agreed • A longitudinal analysis of patient experience across England • A longitudinal analysis of patient experience at Trust, regional and ICS geographic levels • Analysis to be conducted across the patient journey including primary and secondary care. • Analysis to be conducted across GP, Ambulances, A&E, Urgent care centres, 111, and community care • Analysis of continuity of care • Comments to be coded to quality domains • Comments to be coded to CQC location IDs • All analysis to be presented in a slide/graphic format • Benchmarking insights and variation mapped For the maternity pathway, we require • Provision of Contract Data relating to o Women's experience of maternity services on a care pathway, starting with primary care and linking to acute and community care and other care or support services they may use. o Their experiences of their treatment and support o Their experiences of involvement in decision -making o Their experiences of how they are supported outside care environments o All comments must relate to a care provider • Minimum volumes to be agreed • A longitudinal analysis of patient experience across England • A longitudinal analysis of patient experience at Trust, regional and ICS geographic levels • Analysis to be conducted across the patient journey including primary, secondary and community care. • Analysis of continuity of care • Comments to be coded to quality domains • Comments to be coded to CQC location IDs • All analysis to be presented in a slide/graphic format • Benchmarking insights and variation mapped All analysis and reports to be completed in timescales allowing for content to be published in State of Care.
