SSD 18 05 Derbyshire Home from Hospital Support Service

Derbyshire County Council Adult Care are inviting tenders from suitably qualified providers to deliver theDerbyshire Home from Hospital Support Service. This service will provide practical support for vulnerablepeople at the point of hospital discharge, or those in the community at risk of admission to hospital. The service will support people who may not be eligible for social care / NHS support but whose needs are likelyto escalate without any intervention. It will ensure vulnerable people are supported with initial practical supportfor up to 6 weeks to promote confidence and independence.
