PB2022-00634 - Young Person Home Care Package

Home Care Package of Support for Young Person ( 360578). Young Person has complex needs as the result of a medical condition. Previously this service was commissioned by the NHS with 50% of the funding being paid by DBC. The NHS have now assessed owing to the progress made by the young person that they do meet the criteria for Continuing Care, and the reduced care package will now be fully funded by DBC.
