Healthy Lifestyles Service

The London Borough of Merton (LBM) has a requirement for an experienced and qualified organisation(s) to design and deliver healthy lifestyles services for residents/the GP registered population of Merton. As the first step in the Merton prevention framework, this Healthy Lifestyles Service includes four specific components o Outreach, engagement and community resilience o Universal digital offer and gateway o Stop smoking (specialist support, brief support and self care) o Front line training.
